Documents that you provide:
These are records you already have that you upload or submit through secure links. Sometimes a provider's office or outside organization may have these. You can always call and ask them to fax the records to us! Our fax number is 620-206-2514.
ROIs:
A Release of Information (ROI) allows Born to Blossom to communicate with other providers involved in your care. This coordination helps ensure services are aligned, safe, and effective.
We only share information that is necessary and permitted.
We will need the client's insurance plan ROI before we can communicate with them to authorize services and bill them.
Occasionally, we may ask for a general ROI to be completed with a specific outside organizations information such as an SLP, previous therapist, or school so that we can coordinate care.

Contracts:
These forms outline how services are provided, ensure safety, and explain your rights. You have to complete these yearly.
Assessments and Measures:
These assessments help your provider to evaluate your progress and tailor your treatment goals.
